Heavenly Health Care believes in all-inclusive care, which bridges the patient, physician, our team of specialist and family. The key word is bridge. By creating a seamless bond among patient, family and our team of specialists, Heavenly Health Care creates an environment to improve communications to enhance patient care.
By being all-inclusive means talking with the family, providing education and listening to their concerns. This provides the Heavenly Health Care nursing professionals with input to help assess the patient’s needs and provide enhanced care. Family understands the patient’s mental state, pain threshold and may also have insights that the patient may not disclose to our team of specialists.
This information allows our health care professionals to discuss the patient’s care plan with the physician, make alterations to the plan if warranted, or proceed with the prescribed course of action. Patients may relate information to family members or family members may observe situations that are pertinent to the overall care of the patient.
Observations May Include:
- A patient's resistance to taking medications
- Abnormal sleep patterns
- Abnormal speech patterns
- Memory loss
- Blurred Vision
- Lack of appetite
- Lightheadedness
- Dizziness
- Trouble with balance
Our nursing professionals will discuss what to observe based on the patient’s diagnosis, surgery or medications.
Heavenly Health Care suggests that patient observations be written down. If it’s outside your loved one’s normal behavior, write it down. Make sure to include the time, date and what you observed.
During the recovery process, it may take time for your loved one to get back to their normal selves and our staff understands this. However, your notes are always helpful. Also, there may be times our staff may conduct additional assessments based on your notes. This is part of the care process and proper medical protocols.
